Financial

We will be more than happy to submit most insurance forms for you and help you recover the most from your benefits. Forms of payment accepted by the office are check, cash, or Visa, Master Card, Discover, American Express, and Care Credit. Please discuss any financial concerns you may have concerning paying for your services prior to the rendering of such services by our office staff. We will review your situation confidentially and will strive to meet your needs.

New Patient

If you are a new patient, we ask that you provide us with your insurance information upon booking your first appointment.  Prior to your first appointment, we will check your insurance benefits . We will provide you with your benefits information upon your arrival to your initial visit.  If you have not met your deductible and/or have a co-pay, we must collect that amount at the time of your visit.  You also have the option, and we encourage you, to contact your insurance company yourself to find out this information. 

Cancellation

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient.
