We will be more than happy to submit most insurance forms for you and help you recover the most from your benefits. Forms of payment accepted by the office are check, cash, or Visa, Master Card, Discover, American Express, and Care Credit. Please discuss any financial concerns you may have concerning paying for your services prior to the rendering of such services by our office staff. We will review your situation confidentially and will strive to meet your needs.
If you are a new patient, we require you to pay your office visit and treatment at the time of your first visit. Once we can confirm that your insurance covers and pays for such visits and treatments, we will reimburse you for that amount covered by your insurance. For insurances in which we accept assignment, we will bill on your behalf and be paid directly by that insurance company providing you have met your annual deductible and have other insurance that covers co-pays. If you have not met your deductible and/or have a co-pay, we must collect that amount at the time of your visit.
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient.
