If you are a new patient, we ask that you provide us with your insurance information upon booking your first appointment. Prior to your first appointment, we will check your insurance benefits . We will provide you with your benefits information upon your arrival to your initial visit. If you have not met your deductible and/or have a co-pay, we must collect that amount at the time of your visit. You also have the option, and we encourage you, to contact your insurance company yourself to find out this information.
